Physicals required for GHS winter sports

GUILDERLAND — Physical exams will be given to athletes playing winter sports on Oct. 27 from 4 to 5:45 p.m.

Students should sign up at the high school nurses’ office. The physicals are available for students who are new to the school, students who want working papers, and sophomores as well as to athletes.

Winter sports start on Nov. 9 and include boys’ and girls’ basketball, gymnastics, wrestling, bowling, boys’ swimming, cheerleading, ice hockey, cross-country skiing, and indoor track.

These forms need to be completed and returned to the nurses’ office as soon as possible:

— A Department of Athletics Form, completed by a parent or guardian;

— A Health History Form, completed by a parent or guardian; and

— A Physical Examination Form. Students need to come to the nurses’ office to check to see if they have a valid physical. The physical must be completed on a Guilderland High School form.

Students will not be able to try out or participate in practices without a sports packet cleared by the nurses’ office.

If a student is seen by a doctor for any injury, accident, or serious illness, the student’s parent or guardian must notify the school nurse. This is required whether or not the injury or illness occurred on school grounds.
